A revolutionary new way of providing health and care is on the way for Staffordshire and Stoke-on-Trent.
Your Care Connected is a new electronic system which will link NHS and local government organisations more closely than ever before in order to provide the best possible care.
The new digital shared care record means you:
- Will no longer have to repeat information to different health and care professionals
- Can avoid having to undergo unnecessary duplication of tests – leading to fewer delays in receiving the right care
- Empower you to have greater control of your care by enabling you to view your patient record online.
Health and care professionals will also, for the first time, be able to access an individual’s record wherever they are based.
There will be a phased approach to the new system, with the first phase due to launch later this year.
Shared health and care information will include such things as:
- Test results
- Referrals and diagnoses
- Relevant social or mental health information.
Information will be held securely and will only be shared with local health and care professionals involved in an individual’s care, such as a GP, district nurse, social worker or doctor.
Anyone who does not wish to have their information shared in this way will have the option to opt out.
Your Care Connected is being spearheaded by Together We’re Better, the partnership of NHS and local government organisations that is working to transform health and care services in the area.
